We would like to remind you of the disability and leave of absence claim self-service options available through our benefits administrator, Matrix Absence Management (Matrix). Their call center remains available 24 hours a day, 7 days a week by dialing (800) 866-2301. However, in the event of high call volume and increased wait times, you can always file your claims online at www.matrixabsence.com. Whether online or by phone, you’ll receive the same excellent service and care.

How to file your claim online

Log in to your eServices account at matrixabsence.com. Don’t have one? You can create one right from here.

Once you’ve logged in, look at the left side of your screen and click on “File a Claim.” If you have open claims there, look at them to ensure you don’t already have a claim open with the same dates.

From there, it’s simply fill in the blanks. The process should take less than 10 minutes. Don’t have all of the information? That’s okay. You can save your intake form and finish it later.

Here’s what you’ll need to file a new claim:

  • The name, specialty, office phone number and fax number of your treating professional.
  • Important dates like last day worked and when you plan to return to work (best guess
    is fine).
